COVID Vaccine Appts

Appointments can only be made through your web portal account.  Staff are unable to make them by phone.  Login to your portal account and select Appointments –> Book Online Appointments.  For Provider select Visit, Vaccine.  For Visit Type select Covid Vaccine Visit.  The system requires you to enter a reason – you can enter anything you want (e.g. Pfizer).  Enter a preferred date or leave blank for first available.  Then click Search.  Online appointments will be available 20 days in advance.  Parents and Grandparents can get appointments also, but will need to speak to our staff to register and get their own portal account. 

If you have an upcoming well visit, you may also get the vaccine.   If you intend to vaccinate at your well appointment, it would help us plan doses to thaw, if you send a message through your web portal account.  You will also need to print out the forms below for the appointment.

Finally, there is no charge to you for the vaccine. We do bill your insurance company for the administration. This charge is regulated and has no cost share to you. If you are a parent or grandparent, please fill out the following Insurance Information to capture your insurance. Thank you .
