Four Seasons Recognized

Four Seasons Pediatrics has achieved NCQA recognition status for Physician Practice Connections – Patient Centered Medical Home (PPC-PCMH).  Four Seasons Pediatrics was awarded level 3 recognition status for the Patient Centered Medical Home. The Patient Centered Medical Home is a setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

There are nine PPC® standards, including 10 must pass elements, which can result in one of three levels of recognition.

Dr. Miller Comment:Four Seasons Pediatrics is committed to high quality care for our patients. The Patient Centered Medical Home is just one way we show our commitment. We have invested in using technology to help us ensure that our patients get the care that is necessary. We hope that you have seen how we invest in the future of our children. Many of you have noted that we have automated reminder phone calls when you are due for Preventive Exams (check ups), Asthma Reviews, ADHD Appointments, Scoliosis Appointments and labs we have recommended but have yet to be performed. We kindly ask you to respond to these phone calls even if is just to notify us that you are aware and would like to have the reminder come at a different time. We know you are busy and want to respect your time, so feel free to let us know if you want the next reminder at a certain date (e.g. beginning of December). These reminders are part of the Patient Centered Medical Home. The Patient Centered Medical Home is a hot topic in the medical community. Employers, patients, state agencies, the New York State Health Department and insurance complanies are all interested in knowing which physician offices are showing their commitment to excellence.

There will be new enhancements in the 4th quarter of 2010 and in 2011. These will include a new patient portal and an upgrade of our electronic medical record. We are excited about what is coming, and feel that you will continue to be impressed with our new capabilities.
