**Attention: GHI / Emblem Health / HIP insurance will be “Out-of-Network” as of 02/01/2015**

Our practice participates in health plans with the following insurance companies. Participation in these plans may change without notice.

• BlueCross or BlueShield
• CDPHP
• Cigna
• Empire Plan or NYSHIP
• Fidelis (Your child MUST have their own policy, not the parent’s)
• Multiplan or PHCS
• MVP
• Tricare (Military)
• United Healthcare
• If not listed, please call and ask our billing staff

If your plan is Managed Care, such as Child Health Plus or Managed Medicaid, the parent’s policy is NOT the child’s policy.  The parent is responsible for adding the child to their OWN policy.  The insurance will not be under the parent’s policy.

**NOTE:  If your insurance card has “POS” or “Point of Service” printed on the front or back, please contact us prior to making an appointment.  This type of insurance is very specific to which doctors you may see as a patient.
